Held around Mt. Hiei, whose temple history spans some 1,200 years, starting and finishing at Enryakuji parking lot No. 3; the entry fee includes the Enryakuji temple admission. The 50-mile is the longest category: the first loop is the 50km course, and the second repeats part of it.
The course tops out at 846 m (low point 140 m). There are 3 big climbs: 704 m around 15–20 km; 555 m around 26–28 km; 516 m around 49–52 km. Only about 20% is gentle (under 5% grade); most of it is climbing or descending. In 2026, 41% of starters finished — a tough one. The median finish was about 11 h.
