The middle course starts and finishes at Kamabuta Ruins Park in front of JR Joetsumyoko Station. It climbs over Mt. Aota-Nanbayama (949 m), drops to the old Kuwatori road and returns via the Kakuma rice terraces and the old Nakanomata road.
The course tops out at 949 m (low point 24 m). There are 2 big climbs: 925 m around 0–12 km; 376 m around 23–27 km. About 40% of the distance is gentle (under 5% grade).
